How should I prepare for my appointment?

We recommend that you obtain copies of any recent lab, imaging, or other testing that you have had done so that they can be reviewed during your visit. Take some time to gather any family history information including incidence of cancer, diabetes, high blood pressure, thyroid disorders, heart disease, mental health disorders, allergies, etc. Next, reflect on what personal health goals you would like to discuss, as well as any current issues you would like to address during your visit.

One week prior to your appointment, you will receive an email with a request to complete new patient intake forms. Please print and complete these forms and bring them to your visit along with a current insurance card.
